What the ACSM recommendation actually says
The 2009 American College of Sports Medicine position stand recommends a 2 to 10 percent load increase when a person can perform the current workload for one to two repetitions above the desired number at a specified repetition-maximum load.
This is a broad position-stand recommendation, not a command to add the same percentage to every exercise. Two percent on a barbell lift and the smallest available jump on a dumbbell or machine can be very different practical changes. The position stand also says recommendations should depend on goals, physical capacity, and training status.

Randomized trials in trained and previously untrained adults compared increasing load with increasing repetitions. Both methods improved the studied resistance-training outcomes. If the next load jump is too large, repetition progression is not merely a delay. It can be a valid progression strategy inside the program.

No. Progress is not perfectly linear. Holding load can be appropriate when the target is not met, technique changes, reported effort is higher, recovery is poor, or the program intentionally maintains the current demand.
